Training Verification Fee

Physicians who trained in the Anesthesiology or Pain Medicine specialties at the University of Toronto, Department of Anesthesiology and Pain Medicine, may request verification of training within the department. These training verifications are for postgraduate medical education only. Undergraduate studies will not be verified by the department.

Costs Associated with Training Verifications

Please note there are costs associated with training verifications by the Department of Anesthesiology and Pain Medicine.

NOTE: If you are requesting both a residency and fellowship verification, you must submit your request to both programs.

Payments are to be made by credit card, money order, or certified cheque in Canadian or American funds. Certified cheques or money orders are to be made payable to the University of Toronto, Department of Anesthesiology and Pain Medicine.  Personal cheques and cash will not be accepted. United States Postal Service postal money orders will also not be accepted.

If the training verification is requested via mail/priority courier, please note there is an additional cost on top of the verification in addition to the cost of the postal service itself.

Please note: The Department of Anesthesiology and Pain Medicine does not offer notarization services.

Timelines for Training Verification

Training verifications will not be processed until payment has been received and consent to release information has been signed by the trainee and received by the Department of Anesthesiology and Pain Medicine.

Training verifications will be finalized within 10 business days of receiving payment and consent.
